To participate in any program at Pathfinder Farms, all parents must complete a medical release and liability waiver for each of their children. Below is a fillable form, you can download and complete. Please print and present to a Pathfinder Farms staff member, or you may email the completed form back to us at info@pathfinderfarms.org

Release and Hold Harmless Agreement
